4-H Camp Overlook Application

Thank you for your interest in 4-H Camp Overlook. We look forward to having you join us for camp this year. Before you can join us you will need to fill out an application.

All of the documents below are in Adobe Acrobat format. After you download the files, you can open them in Adobe Acrobat Reader and print out, or fill them out on your computer and then print out. Due to Acrobat reader software restrictions you will be unable to save a completed form filled out on your computer. This capability is provided to allow you to type a legible form for sending in if you wish to use.

Get Acrobat ReaderYou will need the free Adobe Acrobat Reader software to view the forms.

To download: If you are using Internet Explorer, right click on each of the links below and select "save target as." If you are using Netscape, right click on each of the links below and select "save link as."

You should not fill in the Acrobat form by opening in your Web browser, as not all browsers and computers will handle them properly. Please download the forms as explained above, then open with Acrobat Reader.

Please complete an Application and Health Form for each camper.  Be sure to read the brochure before completing your application.  Applications that are not complete (including signatures) will be returned and may jeopardize acceptance in your first choice of sessions.  Applications can be sent before the Health Forms but the Health Forms are due by June 16. Applications are accepted on a first come/first served basis according to postal dates.
Return your completed Application to:  4-H Camp Overlook, 355 West Main Street, Malone, NY  12953

A reply newsletter  (which will include the packing list, arrival and departure times, and an invoice )  will be sent to you to confirm your reservation at camp. 

An Invoice will also be sent with this newsletter and is due along with the Health Form by June 16.
